This is an exciting opportunity to join our existing team of Clinical Pharmacists working at Scott Arms Medical Centre on behalf of our PCN Health Vision Partnership.


The successful candidate will be part of a dynamic and forward-thinking team employed by Health Vision Partnership working at Scott Arms Medical Centre. Professional Leadership will be provided by Scott Arms Medical Centre Clinical Lead Partner.


The role is full time, 37.5 hours per work worked over 5 days.


Main duties of the job

The position of Primary Care Network Clinical Pharmacist has an essential role in improving the healthcare we provide to our patients. We are recruiting for a clinical pharmacist to work with existing practice teams to deliver consistency and excellence in medicines optimisation.



  1. Improve patients' health outcomes and efficiency of the practice team by providing direct, accessible and timely medicines expertise, including identifying areas for improvement and initiating change.

  2. Be the primary contact within the practice for queries relating to medicines management, in particular around individual patient care.

  3. Focus on individual patient care by addressing immediate and future needs to personalise care planning.

  4. Proactively transfer workload relating to medicines optimisation from other clinical staff to improve patient care and safety.

  5. Demonstrate the effectiveness of the practice and the role through audit and data analysis.

  6. Follow up patients and investigations, particularly blood tests.

Job responsibilities

The post holder is a clinical pharmacist, who acts within their professional boundaries, supporting and working alongside a team of Health Care Practitioners within general practice and a Primary Care Network (PCN).



  1. Provide primary support to general practice staff with regards to prescription and medication queries.

  2. Perform medicines reconciliation on transfer of care and implement systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the PCN.

  3. Work as part of a multi-disciplinary team in a patient-facing role.

  4. Take responsibility for areas of medicines optimisation within the PCN and undertake structured clinical medication reviews to proactively manage patients with complex polypharmacy.

  5. Provide clinical leadership on medicines optimisation and quality improvement (QI) and manage some aspects of the quality and outcomes framework and enhanced services at practice and PCN level.


Person Specification
Qualifications

  • Mandatory registration with General Pharmaceutical Council (GPhC)

  • May hold or be working towards an independent prescribing qualification.

  • Flexibility to meet the needs of the business

  • DBS required

  • Immunisation status


Skills and Knowledge

  • Excellent written and verbal communication skills

  • Ability to communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome difficult levels of understanding, cultural background and preferred ways of communicating.

  • Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients).

  • Excellent interpersonal, influencing and negotiating skills

  • Ability to effectively plan, manage, monitor, advise and review general medicine optimisations issues in core areas for long term conditions.

  • Recognise priorities when problem solving and identify deviations from the normal pattern.

  • Able to obtain and analyse complex technical information.

  • Knowledge of health promotion strategies.

  • Knowledge of clinical governance issues in primary care.

  • Effective decision-making and negotiation skills.

  • Ability to problem solve and meet deadlines whilst working under pressure.

  • Excellent organisational skills.

  • Work effectively independently and as a team member.

  • Gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action.

  • Demonstrate accountability for delivering professional expertise and direct service provision.

  • IT skills (MS Office, System one and DOCMAN).


Experience

  • Experience working as a clinical pharmacist.

  • Involving patients in decisions about prescribed medicines and supporting adherence as per NICE and local best practice guidelines.

  • In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.

  • An appreciation of the nature of GPs and general practices.

  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.

£39,000 to £46,800 a year Depending on experience
