Locally Employed Doctor (Higher) in Acute Medicine QEHB

This 12-month fellowship post aims to provide doctors with Acute Medicine experience at registrar level in one of the largest Acute Medical Units in the UK while developing skills in research, quality improvement, leadership or education. Candidates will be expected to express an interest in one of these streams upon application.


All applicants are required to have MBBS or an equivalent primary medical qualification and to have completed at least five years of postgraduate training, including three years in a relevant specialty training programme (e.g. IMT). This should include Acute Medicine experience at Tier 2 (SpR) level.


The department is home to the Society for Acute Medicine's Research Lead as well as the national lead of SAMBA (Society for Acute Medicine Benchmarking Audit). Research in Acute Medicine is actively encouraged with NIHR funding supporting ACF posts in recent years. Quality improvement and service development are well-supported by the trust leadership team who have made Urgent and Emergency Care (UEC) one of the trust's four key strategies. Opportunities for teaching are available in our simulation suite and clinical skills area and as part of a thriving FAMUS (Focussed Acute Medicine UltraSound) training programme.


Less-than-full-time (LTFT) working is welcomed. The breakdown of LTFT job plans will be agreed with the successful candidates using a standardised tool to ensure parity between colleagues.


Main duties of the job

The appointee will work under direct supervision of the Acute Medical consultant team to support patients in AMU, ED and MSDEC as described below. They will also be given an educational supervisor and will be encouraged to maintain a portfolio to evidence their clinical learning.


In addition to their clinical work, appointees will be given managerial responsibilities that align with their area of interest (i.e. research, quality improvement, leadership or education). There will also be opportunities to develop subspecialty interests depending on experience and service needs. Job responsibilities

Appointees will work in accordance with the Trust's standard operating procedures and policies. They will commit to attending accredited conferences and meetings to maintain their continuing professional development, in line with national CME requirements, as well as completing mandatory training. The appointee will be accountable to the CSL. The appointee will also participate in an annual job planning and appraisal process as required by the GMC.


Clinical


Responsibilities include ward rounds of our footprint of 94 inpatient beds, assessment of new and follow up patients presenting to MSDEC and a pro-rata contribution to the general medical on-call rota. The appointee will be expected to respond to results appropriately and liaise with other teams as required. The appointee will be accountable for clinical matters to the supervising consultant of the day and will be supported by a senior colleague during all shifts to offer advice as required.


Governance


We have an active governance structure that includes a monthly morbidity and mortality meeting. As well as being expected to attend these meetings, the appointee will recognise and respond to risks and incidents using widely used reporting systems. They will support responses to patient complaints in a courteous and timely manner. They will contribute to audit of departmental and hospital-wide standards as well as contributing to quality improvement work as part of their non-clinical role.


Management


The appointee will be encouraged to adopt management roles within the department. These roles will be mutually agreed following discussion with the CSL after appointment. As above, the appointee will be supported to develop their own areas of interest that contribute to the wider development of Acute Medicine as a speciality.


Person Specification
Qualifications

  • MBBS or equivalent medical qualification

  • Full GMC registration with a current license to practice

  • MRCP Part 1

  • Advanced Life Support (ALS) or demonstration of ability to achieve ALS immediately upon appointment


Experience

  • Demonstrates an interest in and commitment to Acute Medicine

  • Ability to offer expert clinical opinion on a range of acute problems

  • Has completed at least five years of postgraduate training (or equivalent gained on a part-time or flexible basis), including:

  • Completion of a UKFPO-affiliated Foundation programme or evidence of achievement of foundation competencies

  • Three years in a relevant specialty training programme (e.g. IMT)

  • Acute Medicine experience at Tier 2 (SpR) level
