About the Role

We are currently recruiting for an Outreach Navigation Worker to support homeless young people and care leavers who are rough sleeping or at risk of homelessness.

This is a hands-on, young-person-centred role focused on providing information, advice, guidance and practical support to help young people achieve positive and sustainable outcomes.

Key Responsibilities
  • Participate in street outreach to engage with young people who are rough sleeping or at risk of rough sleeping.
  • Provide effective triage, assessment and ongoing support based on individual needs.
  • Support young people to remain in their existing accommodation or secure suitable alternative housing.
  • Work closely with statutory and voluntary agencies to ensure young people's needs are met.
  • Support young people experiencing complex needs, including trauma, poor mental health, offending and substance misuse.
  • Help young people develop positive social networks, resilience and independent living skills.
  • Support young people to move away from the streets and towards stable accommodation and recovery.
  • Maintain accurate records and contribute to positive outcome-focused support planning.
About You

We are looking for someone who:

  • Has experience working with young people - essential.
  • Has experience supporting individuals with complex needs and challenging circumstances.
  • Is resilient, compassionate and able to build positive relationships.
  • Can work independently and use their own initiative.
  • Is flexible and able to respond effectively to changing situations.
  • Is comfortable working outdoors and undertaking significant walking as part of street outreach.
  • Can work collaboratively with partner agencies and professionals.
  • Is committed to helping young people achieve independence and sustainable outcomes.

Driving: Full UK driving licence and access to a vehicle is desirable but not essential.
