Enhanced PCN nurse will support MDT working, e-triage, and developing new pathways for Apollo WN PCN.


We are looking for an adult-trained nurse who is eager to develop new skills and seek a new challenge working alongside our clinical team to support online consultations and our new neighbourhood team.


Main duties of the job

  1. Facilitate and support the development and delivery of innovative training and education to staff.

  2. Provide professional and clinical leadership support in development and delivery of work streams related to area of expertise as part of the PCN's strategy.

  3. Work proactively with key local and national stakeholders to develop more integrated care pathways and holistic models of care; consult with patients, their families, and the multi-professional team to undertake assessments of patient needs and devise and evaluate complex care plans.

  4. Deliver enhanced clinical care in the context of continual change, challenging environments, different models of care delivery, innovation, and rapidly evolving technologies.

  5. Advise patients and their families on managing their conditions.

  6. Participate in clinical audits and research projects and implement necessary changes.

  7. Work within national and local protocols, recognize boundaries of practice, know when and to whom patients should be referred, delegate work responsibly, and demonstrate initiative and creativity in problem-solving.

  8. Hold responsibility for team performance and service delivery; provide supervision to trainee nurse associates, nurse associates, and general practice nurses.

Job responsibilities

The Enhanced PCN Nurse will work autonomously and as a vital member of a multidisciplinary team, delivering high-quality, proactive care for patients within the Primary Care Network. This includes remote management via our e-Hub and face-to-face assessments in practices, care homes, and patients' homes. The postholder will provide expert nursing care across acute and chronic conditions, support vulnerable groups, and reduce avoidable hospital admissions through integrated, personalized care.


Key Responsibilities
1. Clinical Care and Management

  • Assess and manage patients with acute and long-term conditions such as skin conditions, respiratory issues, contraceptive and HRT support, urinary tract infections, and minor ailments.

  • Provide consultations via telephone, video, and in-person using the digital e-Hub model.

  • Prioritize care needs safely and effectively.


2. Mental Health and Holistic Support

  • Support patients with mental health concerns, referring as appropriate.

  • Manage patients with autism, learning disabilities, and serious mental illness holistically.

  • Engage in care planning and health promotion.


3. Long-Term Condition Management

  • Implement care plans for chronic diseases like diabetes, hypertension, COPD, asthma, and heart failure.

  • Support patients in managing their health.


4. Community & Home Visiting

  • Conduct proactive visits to housebound patients, care homes, or those at risk of hospital admission.

  • Perform holistic assessments, identify unmet needs, and coordinate care.

  • Collaborate with MDT members and care coordinators.


Diagnostics and Procedures

  • Perform procedures like ECGs, blood pressure monitoring, sepsis screening, phlebotomy, and specimen collection.

  • Request and interpret results for treatment planning.


Professional Responsibilities

  • Maintain accurate clinical records.

  • Use appropriate coding and templates.

  • Ensure safe prescribing within scope.

  • Participate in audits and quality improvement.


Education and Mentorship
  • Complete induction, participate in CPD, and mentor junior staff.

Collaborative Working

  • Act as a link between practice, community, secondary care, and voluntary sectors.

  • Support PCN strategies and promote self-care.


Person Specification
Qualifications

  • NMC Registered Nurse (Level 1 Adult)

  • Experience in Primary/Community Care

  • Proficient with EMIS or similar systems

  • Assessment and decision-making skills

  • Autonomous and team working skills

  • Commitment to person-centered care

  • Previous PCN or integrated care experience

  • Knowledge of Quality Improvement methodologies

  • Experience with vulnerable populations

  • Experience in care planning


Additional Information

  • Travel across the PCN footprint; full UK driving license and vehicle essential.

  • IT equipment provided.

  • Role aligns with NHS Long Term Plan and related initiatives.
