About the role
To provide early and targeted support to aid the recovery and resilience of people with multiple vulnerabilities. You will work across a portfolio of longer-term accommodation-based services aimed at addressing the 'revolving door' nature of short-term supported accommodation. Demonstrating a commitment to safeguarding in line with policy and procedure, you will support customers to develop the knowledge and skills they need to achieve their goals, empowering them to build successful relationships within their community, access necessary services, and develop skills to better manage their health and wellbeing, thereby increasing their independence as they progress through a prevention pathway model.

About You
We seek individuals with relevant experience in providing specialist support, covering activities such as:
- Excellent communication skills and a customer-focused approach
- Effective collaboration with a wide range of stakeholders, statutory and non-statutory
- Experience working across multiple locations to support a caseload of customers
- Facilitation of customer involvement
- IT literacy, including Word, Excel, Support Planning Software, and database management
- Maintaining accurate records to capture outcomes and KPIs
- Ability to manage competing priorities and meet deadlines
- Flexibility to work within a shift pattern environment
- An empathetic nature and passion for supporting vulnerable adults
