Role: Qualified Social Worker - Transitions Team Type: Locum, full time hour Location: Birmingham Rate: £28 an hour Eden Brown are looking to appoint an enthusiastic adults social worker on behalf of Birmingham City Council. The vacancy is working within the transitions team where youll be working with young people between the ages of 18-25. Day to day duties include ill require you to undertake conversation 3 discussions support planning, commissioning of care forms (budget requests), reviews, mental capacity assessments, attend meetings for: best interest EHCP, section 117 reviews complete CHC checklists as part of your case management. You will be fully supported by Senior Practitioners and the assigned Team Manager and you will be working on a hybrid basis. To apply for this role you must: - Be a qualified Social Worker - Have a minimum of 2 years post qualified experience. - Have an enhanced DBS certificate. - Experience working within a transition team would be advantageous.
