The Role: As a Social Worker in Bristol's Adult Services, you'll play a crucial role in assessing and addressing the diverse needs of our community, navigating the varied challenges our vibrant city provides. Your responsibilities will include:




  • Conducting assessments and developing tailored care plans for service users and carers, ensuring that their unique needs and priorities are addressed.

  • Advocating for choice and control, you will empower individuals to make decisions and manage risks, ensuring services are organised in alignment with their preferences and needs.


  • Uphold professional standards continuously; as our service evolves, you’ll monitor changing needs, develop support plans, utilise resources, and ensure Social Care remains future-ready through creative use of assistive technology.


Key Responsibilities:




  • Person-Centred Care: Develop and implement care plans that are individualised, considering the person’s strengths, preferences, and goals.


  • Assessment and Intervention: Conduct thorough assessments of service users, develop tailored support plans, and facilitate access to necessary services.


  • Collaboration: Work closely with other professionals and agencies to provide coordinated care and ensure the well-being of those you serve.


  • Advocacy: Advocate for service users to ensure their voices are heard and their rights are protected, ensuring they have choice and control over their care and support.


  • Safeguarding: Ensure that individuals who are at risk are safeguarded, making decisions and taking actions to protect vulnerable adults.


You Will:



  • Be a qualified social worker with Social Work England registration and possess a Social Work degree or equivalent qualification.

  • Have a passion for person-centred Social Care and a proven ability to involve people in decision-making, building on their strengths.

  • Be highly motivated and adaptable, with a collaborative spirit for building relationships and working in diverse environments.

  • If applying for the post-qualification role, you will hold a Post Qualification Award (e.g., PEPS 1&2, Deaf Blind Assessors, BIA, or AMHP), and have significant experience in the relevant area.
